Consent form
同意書 dōisho
Written consent from a parent, legal representative, payer or another affected person when the contract or procedure requires their approval.
A consent form and an authorization letter serve different purposes. Consent means 'I approve this application or use'; authorization means 'this person may act for me'. A minor's contract, filtering-service decision, use of another person's payment method or certain family procedures may require one or both.
Bring the latest carrier form and the identity documents specified for the consenting person. Missing handwriting, dates, signatures, seals or contact details can stop the procedure.
