Authorization letter
委任状 ininjō
A document written or signed by the contract holder authorizing a named proxy to perform specified procedures.
Use the carrier's latest form whenever possible. The holder normally completes it, identifies the line and exact procedure, names the proxy and signs or seals it. Forms can expire; SoftBank, for example, states three months from creation and may require one form per target number.
An authorization letter does not make every procedure available to a proxy. A carrier may prohibit proxy handling for a PIN change, detailed call records or another security-sensitive task, and may telephone the holder to confirm the request.
